City of Albuquerque: Focused Data Driven Interventions

 

 

In August 2003, PHP began providing quarterly clinical analysis reports to the City of Albuquerque. The clinical reports are a core element of the Presbyterian’s Healthy Advantage program designed to improve the health of employees through high touch, high tech interventions. The clinical quarterly report consists of: health care cost drivers, comparative data with PHP commercial and large employer groups, and recommended areas of focus.

 

The City provided us the PHP membership breakdown by job category in order for us to determine the disease prevalence by job category.

PHP suggested to the City that we had the opportunity to provide onsite cholesterol screenings among the service sector. The City contacted the various service departments and determined that Solid Waste Management would participate in onsite cholesterol screenings. The screenings dates were June 22 and June 29, 2005 at three sites: Pino Yards, Cerro, and Edith. Posters in both Spanish and English were posted throughout all the three locations two weeks before the screenings. Seventy-nine employees participated in the screenings. They were each sent a cover letter with their test results. PHP and Lovelace members sent follow-up communications.
